Once you find out you are pregnant, please fill out the form through the link below. You can either refer yourself to Bedford or Luton & Dunstable Hospital. They wish to see all women before 10 weeks of pregnancy. Once you choose who you wish to be referred to, please complete the form and wait to hear from the assigned community midwife with your first appointment with them
